CSCDOP (Carrier Street Clinic Disability Outreach Project)

The Carrier Street Clinic in Benalla, supported by Murray Primary Health Network, has introduced an outreach service, aimed at people who are unable to access health care because of a disability, and designed to help them to be able to get long term health care, by working with existing services. This service is free and will initially involve those people living in Benalla.

Eligible consumers are those

  • Who are unable to access primary health care services, and
  • Has intellectual, developmental and or physical disability
  • Affected by complex or chronic disease/s and/or
  • Who need early intervention and care planning

It will include an outreach service, so people can be seen in their homes, or the place of their choice, and will include access to computer software to allow people to contact health workers either by video or telephone link. This will also allow storage of important health documents and the workers and others who can access the documents is controlled by the person with the disability or their family.
