The Carrier Street Clinic in Benalla, supported by Murray Primary Health Network, has introduced an outreach service, aimed at people who are unable to access health care because of a disability, and designed to help them to be able to get long term health care, by working with existing services. This service is free and will initially involve those people living in Benalla.
Eligible consumers are those
- Who are unable to access primary health care services, and
- Has intellectual, developmental and or physical disability
- Affected by complex or chronic disease/s and/or
- Who need early intervention and care planning
